In consideration of the participant attending Harvest Adventure Camp, the undersigned parent/guardian hereby releases and holds harmless Harvest Adventure Camp, its teachers, officers, employees, and agents (hereinafter collectively referred to as "Harvest") from any and all liability, damage, cost, or expense occurring during the participation of the undersigned child in this program, even if that liability may arise out of negligence or mistakes on the part of the persons or entities mentioned above.

In particular, the undersigned parent/guardian acknowledges that s/he and such child will not hold Harvest liable for any expenses, property damages, personal injuries, illness, and/or death sustained by such child while participating in the program. Furthermore, the undersigned parent/guardian acknowledges that s/he is, prior to the commencement of Harvest Adventure Camp, aware of and understands the risks involved in this activity, and is prepared to assume, on behalf of such child and himself/herself, all of such risks as his/her and the child's sole responsibility.

The Harvest Adventure Camp staff promise to do their very best to always be vigilant and keep your children safe from illness and serious injury; however, there are inherent risks involved in our full-immersion nature enrichment program for children. It is possible that your child will experience risks like insect bites or stings, poison ivy rash, scratches, scrapes, and bruises. It is possible that despite our vigilance, children will not maintain the proper COVID19 social distance or other safety measures laid out in the Harvest Adventure Camp Policies and Procedures. The possibility thus exists for more severe illness or injuries, including death. We will do our very best to carry out our health and safety protocols with rigor, using frequent multi-sensory training, reminders, and redundant supervision. Likewise, we urge children to take ownership, be considerate, and think about how their actions might affect others. We are a group of responsible community members responding sensibly after conducting research based on best global practices and multi-level stakeholder consideration. Again, we promise to do our very best.

I acknowledge that I am signing this document freely and voluntarily and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
Medical Release and Authorization: As Parent and/or Guardian of the named camper, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the minor’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed. Permission is hereby granted to the attending physician to proceed with any medical or minor surgical treatment, x-ray examination and immunizations for the named athlete. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me. Permission is also granted to the . and its affiliates including Directors, Coaches, and Team Parents to provide the needed emergency treatment prior to the child’s admission to the medical facility. Release authorized on the dates and/or duration of the registered season. This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.
Confirmation: BY ACKNOWLEDGING AND SIGNING, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
